TMZ reports that toxicology reports indicate that former Stone Temple Pilots frontman Scott Weiland OD'ed from a combination of alcohol, cocaine, and ecstasy. Weiland also had heart disease and asthma. At the time of his passing, his ex-wife wrote a letter which was reprinted by Rolling Stone. Read the full letter here.
